Community Forward SF is seeking a Medical Respite Driver to transport patients to appointments and essential services, assist with ADLs, and help navigate to destinations within the Medical Respite Program. The role requires a valid CA driver’s license and a good driving record.

The position emphasizes safety, communication, and coordination with on-site staff and community partners. Requires TB clearance and CPR/First Aid within specified timelines.

Formación

• High school diploma or GED required.
• Valid California Driver’s License with good driving record.
• At least 1 year of experience driving a commercial passenger vehicle.
• Ability to work independently and as part of a team.
• CPR and First Aid certification within 90 days of hire.
• TB clearance within 7 days of hire.

Responsabilidades

• Transport patients/clients to and from appointments and essential services.
• Assist patients/clients to board/disembark and navigate to on-site destinations.
• Maintain daily logs of transports and referrals.
• Coordinate with Medical Respite staff and other service providers.
• Provide safe, courteous transportation and assist with ADLs as needed.
• Maintain and clean assigned vehicles.
