HonorHealth in Phoenix, AZ is seeking a Pharmacy Medication Reconciliation Technician to improve medication safety by assisting pharmacists in collecting and documenting up-to-date medication lists for admitted patients.
You will verify histories in the electronic medical record, document medication lists, and communicate issues to pharmacists and providers, with responsibilities including other duties as assigned.

JOB SUMMARY The Pharmacy Medication Reconciliation Technician will improve medication safety by assisting pharmacists in collection and documentation of up‑to‑date medication lists for patients being admitted to the organizations hospitals in order to optimize the medication reconciliation process and reduce the number of medication errors secondary to inaccurate or incomplete information.
Responsible for obtaining and verifying medication history in the electronic medical record in an accurate and timely manner in compliance with standard procedures and regulatory standards.
Responsible for accurately documenting medication lists into the electronic health record, communicating to the verifying pharmacist, and relaying any outstanding transition of care issues to the pharmacist and/or provider in a timely manner.
Performs other duties as assigned.
High School Diploma or GED Required Other / Certificate Completion of a formalized Technician Training Program. Preferred
>10 months pharmacy technician experience or completion of a formalized technician training program. Required Retail pharmacy experience and/or excellent customer service skills Preferred
Certified Pharmacy Technician (CPhT) - Certification Current license by AZ State Board of Pharmacy. Required Certified Pharmacy Technician (CPhT) - Certification Pharmacy Technician Certification Board (PTCB) Current Pharmacy Technician Certification (CPhT) issued by Pharmacy Technician Certification Board (PTCB). Required Pharmacy Technician (LPT) - License Pharmacy Technician Diploma Preferred
