Theoria Medical is seeking a Medical Director and Attending Physician for multiple skilled nursing facilities. This full-time role emphasizes relationship-based care, flexibility over schedules, and a supportive clinical team to focus on patient-centered primary care.

The ideal candidate will hold an active U.S. medical license, be board-certified in a relevant specialty, and commit to high-quality care with ongoing professional development and incentives.

Responsabilidades

• Serve as attending physician for patients in assigned SNF facilities.
• Collaborate with NPs/PAs to manage daily patient care and supervise clinicians.
• Provide high-quality, patient-centered primary care, diagnosing and treating acute and chronic conditions.
• Participate in chronic care management, transitional care, remote patient monitoring, and other quality initiatives.

What You’ll Do

• Serve as the attending physician for patients in each assigned SNF.
• Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
• Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
• Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
  

Your Qualifications

Physicians (MD/DO) must hold an active U.S. medical license and be board-certified in an appropriate specialty. New graduates who are board-eligible and actively pursuing licensing are encouraged to apply. Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
