Job summary

Medical Home Network is seeking a dedicated Medical Assistant to support Medicare AWVs in community health centers. You will prep patients, assist providers with geriatric assessments, and document histories and risk factors in the EMR.

This on-site role involves coordinating care and ensuring preventive services are completed, while educating patients on wellness. Ideal candidates have MA certification, strong organizational skills, and experience with EMR systems.

Qualifications

  • Requires MA certification and high school diploma.
  • Experience with AWVs or preventive care is preferred.
  • Familiarity with EMR/EHR systems and geriatric care is a plus.

Responsibilities

  • Prepare patients for Medicare Annual Wellness Visits (AWVs).
  • Assist providers with geriatric-focused assessments and preventive care.
  • Document patient history, meds, and risk factors in EMR.
  • Take vital signs and basic measurements (height, weight).
  • Coordinate care and ensure recommended screenings are completed.
  • Educate patients on preventive care and follow-up services.

THE OPPORTUNITY:

MHN is seeking a dedicated and compassionate Medical Assistant to support Annual Wellness Visits (AWVs) for Medicare patients in community health center settings in the Springfield, IL area. This role offers a unique opportunity to advance preventive, geriatric-focused care for seniors in under-resourced communities.

In this position, you will help bring specialized geriatric expertise into community health centers that traditionally do not have dedicated geriatric services, improving both the level and quality of care delivered to older adults. The Medical Assistant plays a critical role in preparing patients for visits, supporting providers, and ensuring comprehensive wellness assessments that focus on prevention, early detection, and chronic condition management.

This is an onsite role working directly with patients and care teams to ensure high-quality, patient-centered care for aging populations. This role will work onsite at our partner health center locations across Springfield and Decatur, IL.

  • Onsite: Monday–Thursday
  • Remote: Friday for administrative work
WHAT YOU CAN LOOK FORWARD TO:
  • Prepare patients for Medicare Annual Wellness Visits, including intake, documentation, and vital signs.
  • Assist providers with geriatric-focused assessments, screenings, and preventive care services.
  • Collect and document patient medical history, medications, and health risk assessments in the electronic medical record (EMR).
  • Perform basic clinical tasks such as:
    • Vital signs
    • Height/weight measurements
    • Vision screenings
    • Fall risk assessments
    • Cognitive and depression screenings
  • Coordinate care activities and help ensure completion of recommended preventive services and screenings.
  • Support providers in identifying care gaps and opportunities for preventive interventions.
  • Educate patients on preventive care, wellness planning, and follow-up services.
  • Maintain accurate and timely documentation in compliance with clinical and regulatory standards.
  • Collaborate with physicians, nurse practitioners, care coordinators, and clinic staff to deliver a team-based care experience.
  • Ensure exam rooms and clinical supplies are stocked and prepared for patient visits.
WHAT YOU’LL NEED TO SUCCEED:
  • High school diploma or equivalent required.
  • Medical Assistant certification (CMA, RMA, or equivalent) required
  • 2 years of experience as a Medical Assistant in a clinical setting preferred.
  • Experience working with Medicare patients or geriatric populations strongly preferred.
  • Familiarity with electronic medical records (EMR/EHR) systems.
  • Strong organizational and communication skills.
  • Compassionate, patient-centered approach to care.
  • Ability to work collaboratively in a fast-paced healthcare environment.
  • Experience supporting Annual Wellness Visits (AWVs) or preventive care visits.
  • Experience working in community health centers, FQHCs, or underserved populations.
  • Knowledge of Medicare preventive services and screenings.
