Habitat Health is seeking a Medical Assistant to support the PACE program, delivering clinical and administrative support in the center and at participant homes. This role partners with providers and the IDT to ensure smooth participant flow, accurate documentation, and timely care.
Responsibilities include intake, vital signs, phlebotomy, immunizations, and coordinating follow-up visits within a fast-paced environment.

Role Scope : The Medical Assistant supports the delivery of high-quality, participant-centered care by providing clinical and administrative support within the PACE center. This role partners closely with providers and the Interdisciplinary Team (IDT) to support participant flow, perform routine clinical procedures within scope of practice, maintain accurate documentation, and communicate changes in participant status. Through reliable follow-through, attention to detail, and a collaborative approach, the Medical Assistant contributes to a safe, efficient, and welcoming care experience for participants.

The anticipated base salary range for this position is $22 - $28/hour . This position is also eligible for a target annual bonus of 5% and may be eligible for additional incentive compensation, as applicable.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
