The role involves assisting with resident and family support services, coordinating social services and discharge planning, maintaining accurate records, communicating with the care team, and assisting with Medicaid-related processes.

Qualifications

• Strong communication, organizational, and interpersonal skills.
• Basic knowledge of the Medicaid process.
• Skilled nursing facility or long-term care experience preferred.
• Social services experience preferred; a degree in social work or a related field is not required.

Responsibilities

• Assist with resident and family support services.
