Job summary

Avamere is seeking a part-time Medication Tech to administer medications and provide direct care to residents at The Stafford in Lake Oswego, OR. The shift is Sundays, 6am-2pm, with supervision by a licensed nurse and adherence to facility policy.

Responsibilities include recording medications, ensuring accurate administration, and communicating with pharmacy and care teams. Candidates should have EMR experience and CPR/First Aid certification, be 18+, and demonstrate warmth and professionalism

Qualifications

• Experience as a Caregiver and Medication Tech in a Memory Care Unit and/or Assisted Living Facility.
• Must have CPR/First Aid Certification during employment.
• Must have Food Handler’s Card.
• Experience in electronic medical records documentation.
• Must be 18 years or older.
• Friendly, attentive, and professional demeanor with residents and guests.
• A genuine desire to improve quality of life for seniors.

Responsibilities

• Pass medications per physician orders under supervision of a licensed nurse.
• Administer medications accurately and timely.
• Record medications in the resident’s record.
• Notify pharmacy or family when refills are needed.
• Maintain a clean medication cart and room.
• Monitor and report medication side effects to Health Services Director.
• Record shift information and changes to orders in EMR.
• Follow care plans for each resident’s needs.
• Assist with medical appointments and communication with providers.
• Observe residents for health changes and monitor vital signs.
• Document changes in status in the EMR.
• Complete incident reports as required.
• Perform delegated nursing tasks as assigned by the RN.
• Participate in in-service education.
• Report changes in resident conditions promptly.
• Follow written and oral directions.
• Maintain resident safety and confidentiality.
• Work with staff, families, and providers.
• Help train new employees on policies.

Medication Tech

Status: Part-Time

Schedule: Sundays only, Day shift, 6am-2pm

Location: Avamere at The Stafford- 1200 Overlook Drive, Lake Oswego, OR 97034

The primary role of the Medication Tech is to administer medications and provide direct care to residents.

Essential Duties and Job Responsibilities

• Pass medications according to physician orders, delegated by and under the supervision of a licensed nurse and in accordance with facility policy.
• Administer accurately and timely.
• Record medications administered in the resident’s record.
• Notify the pharmacy (or family when applicable) when medications need to be refilled.
• Maintain a clean and organized medication cart and medication room.
• Monitor and report any/all side effects of medications to the Director of Health Services.
• Obtain/provide shift report. Record sufficient information related to medication administration to implement accurate changes to medication orders.
• Follow comprehensive care plans related to medication administration based on each resident’s needs and their comprehensive assessment as assigned.
• Assisting with medical appointments, communicating with healthcare providers, and advocating for the residents’ needs
• Observing residents for changes in health, reporting concerns to medical professionals, and monitoring vital signs.
• Document information pertaining to changes in residents’ status, personal needs, and communications with outside providers in the electronic medical record.
• Complete accident and incident reports in a timely manner and according to facility policy
• Perform delegated nursing services as assigned by the RN.
• Participate in in-service education programs as assigned.
• Promptly report changes in resident conditions to the Director of Health Services.
• Follow written and oral directions.
• Perform all duties assigned in an effective, cooperative, timely and professional manner.
• Be responsible for the safety of residents under their supervision.
• Work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers.
• Assist new employees in following established facility policies and procedures.
• Maintain confidentiality of resident and facility information.
• Perform other duties as assigned by the manager or supervisor.

• Must have the ability to obtain a Food Handler’s Card.
• Experience in electronic medical records documentation.
