Job summary

Qualifications

• Proficient in clinical assessment and documentation.
• Knowledge of pharmacology and nursing best practices.
• Strong interpersonal, communication, and organizational skills.
• Ability to function in high-stress situations with diverse patient populations.
• Familiarity with EMR and clinical documentation systems.
• Ability to lift up to 50 lbs and perform extended standing and mobility.

Responsibilities

• Perform comprehensive assessments and reassessments to determine nursing care needs.
• Develop, implement, and evaluate individualized plans of care.
• Administer medications and treatments per policy and physician orders.
• Document assessments, interventions, outcomes, and education timely and accurately.
• Provide direct care including ADLs, hygiene, nutrition, and mobility support.
• Monitor patient conditions and communicate changes to the care team.
• Collaborate with interdisciplinary team for care coordination and discharge planning.
• Provide emotional and psychosocial support to patients and families.
• Ensure patient safety with infection control and fall prevention protocols.
• Supervise and delegate tasks to licensed and unlicensed staff.
