Qualifications

• RN license in good standing (US).
• Associate's Degree in Nursing required.
• BLS certification; ACLS and NIHSS as applicable.
• 1+ acute care experience preferred.

Responsibilities

• Assess, develop, implement, and evaluate patient care plans with a multidisciplinary team.
• Conduct thorough patient assessments and plan discharge needs.
• Serve as liaison to post-hospital care providers and community resources.
• Oversee patient transitions and document care in the EMR.
• Respond to emergencies and maintain infection control standards.
