Job summary

St. Bernards hospital is seeking a Medication Reconciliation Pharmacy Technician to independently obtain medication and allergy histories for patients admitted to the facility.

You will interview patients and families, contact local pharmacies, and use additional resources to gather information essential for safe care. You will input data into the hospital computer system for physicians and clinicians to determine treatment plans and discharge medications, working in a team to ensure accuracy

Qualifications

• High school diploma or GED required and Arkansas license as a pharmacy technician or LPN before work starts.
• National certification for pharmacy technicians is preferred.
• Acceptance into a professional post-graduate program or a minimum of 2 years of relevant experience is acceptable.

JOB REQUIREMENTS

• Education
  • High school diploma or GED required. Must be licensed with the Arkansas State Board of Pharmacy as a pharmacy technician or have an LPN license before the onset of duties. National certification for pharmacy technicians is preferred, acceptance into a professional post graduate program (medicine or pharmacy), or a minimum of 2 years experience.
• Experience
  • Excellent verbal communication skills, ability to work in a team environment and ability to work in stressful environments is essential.
  • The ability to effectively prioritize and manage multiple tasks is also desired.
  • Pharmacy/healthcare experience with direct patient care roles is preferred.
• Physical
  • This is a safety sensitive position. Please see the St. Bernards Substance Abuse Policy for further information.
  • Normal hospital environment. Close eye work. Hearing within normal range. Operates computer, typewriter, calculator. Finger dexterity. Eye-hand coordination. Frequent sitting, standing and walking. May lift and carry up to 35 lbs.

JOB SUMMARY

A Medication Reconciliation Pharmacy Technician will independently obtain medication and allergy histories for patients that will be admitted to the hospital. Information will be obtained by interviewing the patient, patient family, calling local pharmacies and also using other available resources. The information obtained will be entered into a computer system and it will be used by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital.
