Position: MDS Coordinator, RN (Full Time)

What You'll Do

As an MDS Coordinator / Nurse Assessment Coordinator, you will complete and ensure the accuracy of Minimum Data Set (MDS) assessments for all residents. Your role contributes to personalized care plans and ensures proper clinical reimbursement.

Key Responsibilities

• Determine Patient Driven Payment Method (PDPM) and assess expenses related to admissions
• Participate in admitting prospective residents, assessing nursing needs, and determining reimbursement levels
• Complete and verify the accuracy of MDS processes for all residents
• Monitor Case Mix Index (CMI) scores for potential risks affecting reimbursement
• Ensure revenue integrity and compliance with state and federal regulations for MDS coding
• Collaborate with interdisciplinary teams for accurate assessment data collection
• Provide education and insights to facility staff and leadership
