Qualifications

• Strong knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning.

MDS Director (RN/LPN)

Facility: Melbourne Healthcare and Rehabilitation

Location: Melbourne, FL

Schedule: Full-Time | Tuesday–Friday | 10-hour shifts | 6:00 AM–4:30 PM

Compensation: $95,680–$101,920 per year, based on experience

Melbourne Healthcare and Rehabilitation is seeking an experienced and highly organized MDS Director (RN/LPN) to lead our facility’s MDS program and ensure accurate, timely, and compliant assessments and documentation.

This is a key leadership position responsible for supporting clinical documentation, care planning, regulatory compliance, and appropriate reimbursement while working collaboratively with an interdisciplinary team to promote excellent resident outcomes.

Key Responsibilities

• Oversee the accuracy, completeness, and timeliness of all MDS assessments and supporting documentation
• Ensure compliance with applicable state and federal regulations, including Medicare and PDPM requirements
• Conduct comprehensive resident assessments and interviews
• Develop, implement, and monitor individualized care plans for short-term and long-term residents
• Lead and participate in interdisciplinary care plan meetings
• Collaborate with nursing staff, physicians, therapists, specialists, residents, and families to support positive resident outcomes
• Provide leadership, guidance, and education to nursing staff
• Conduct in-service training related to MDS, RAPS, triggers, PDPM, Medicare guidelines, and care planning
• Maintain and manage MDS and care plan schedules for all residents
• Monitor assessment and documentation processes to ensure regulatory compliance and timely completion
• Partner with facility leadership and interdisciplinary departments to identify opportunities for improved clinical outcomes and documentation

Qualifications

• Active, unencumbered Florida RN or LPN license

• Strong leadership, organizational, communication, and time-management skills
• Ability to manage multiple deadlines while maintaining accuracy and regulatory compliance
