Job summary

Diversicare is seeking an MDS Coordinator (RNAC) in Oak Ridge, TN to oversee RAI processes, ensure accuracy and regulatory compliance, and coordinate care plans with the interdisciplinary team.

The role requires an active RN license in the state and knowledge of MDS 3.0, with candidates often having MDS experience. This on-site position offers a competitive benefits package and opportunities for leadership.

Qualifications

• Current registered nursing license in the state of employment.
• MDS 3.0 knowledge and regulatory familiarity.
• Two years of MDS experience preferred.

Responsibilities

• Coordinate the RAI Process for accuracy and regulatory compliance.
• Collaborate with interdisciplinary team to assess needs and plan care.
• Conduct Care Plan conferences with patients, residents and families.
• Provide education related to the RAI process and MDS coding.
• Monitor Quality Measures and ensure MDS accuracy.
• Maintain documentation and timely submission to state databases and entities.
• Ensure compliance with Medicare and Medicaid guidelines.
