Job summary

Diversicare is seeking an MDS Coordinator (RNAC) to ensure accurate MDS assessments and regulatory compliance for our Foley facility near Pensacola. The role involves leading RAI processes, collaborating with an interdisciplinary team, and conducting care plan conferences with patients and families.

The ideal candidate has RN licensure and working knowledge of MDS 3.0, with the ability to educate staff and maintain meticulous documentation to support quality measures and regulatory requirements.

Qualifications

• Two years of MDS experience preferred.
• Current RN license in the state of employment.
• Working knowledge of the MDS 3.0.

Responsibilities

• Coordinate the RAI process, ensuring accuracy and compliance with state and federal regulations.
• Collaborate with the interdisciplinary team to assess patient/resident needs and coordinate care plans.
• Conduct care plan conferences with patients, residents, and families.
• Provide education related to the RAI process and ensure accurate coding of MDS assessments.
• Monitor Quality Measures and ensure MDS accuracy to reflect quality standards.
• Maintain accurate documentation and ensure timely submission to state databases and other entities.
• Ensure compliance with Medicare and Medicaid regulatory guidelines.
