Job summary

Diversicare Healthcare Services, LLC is seeking an MDS Coordinator (RNAC) to oversee RAI accuracy and regulatory compliance across the care team in our facility. This role emphasizes coordinating assessments, coding accuracy, and collaboration with the interdisciplinary team to support resident care plans.

The ideal candidate has MDS knowledge, an active RN license in the state of employment, and familiarity with MDS 3.0.

Qualifications

• Two years of MDS experience preferred, but not required.
• Current RN license in the state of employment.
• Working knowledge of the MDS 3.0.

Responsibilities

• Coordinate the RAI process for accuracy and regulatory compliance.
• Collaborate with the interdisciplinary team to assess resident needs and plans.
• Conduct care plan conferences with residents and families.
• Provide education on the RAI process and correct MDS coding.
• Monitor quality measures and ensure MDS reflects quality standards.
• Maintain documentation and timely submission to state databases and entities.
• Ensure compliance with Medicare and Medicaid guidelines.

Responsibilities

• Coordinate the RAI Process, ensuring accuracy and compliance with state and federal regulations.
• Collaborate with the interdisciplinary team to assess patient/resident needs and coordinate care plans.
• Conduct Care Plan conferences with patients, residents, and families.
• Provide education related to the RAI Process and ensure accurate coding of MDS assessments.
