Job summary

Diversicare Batesville, MS is seeking an experienced MDS Coordinator to lead RAI processes and work with an interdisciplinary team to ensure compliant MDS assessments.

Qualified candidates hold an active Mississippi RN license and have MDS 3.0 knowledge, with Long Term Care experience preferred. The role requires accurate coding, timely documentation, and coordination of care plans with patients, residents, and families.

Qualifications

  • Must hold current registered nursing license in the state of employment.
  • Two years of MDS experience preferred.
  • Experience in Long Term Care preferred.
  • Proficient in Data Entry and Word processing; knowledge of MDS 3.0.
  • Knowledge of nursing practices and related regulations.

Responsibilities

  • Coordinate the RAI Process.
  • Collaborate with interdisciplinary team to assess patient/resident needs.
  • Participate in Care Coordination meetings.
  • Ensure accurate and timely MDS assessments per regulations.
  • Code MDS assessments from records and interviews.
  • Maintain MDS assessment schedules and due dates.
  • Coordinate Care Plan conferences with team, patient, and families.
  • Submit required documentation to state databases per guidelines.
  • Ensure Medicare/Medicaid regulatory guidelines are met.

Responsibilities
  • Coordinate the RAI Process.
  • Work in Collaboration with the interdisciplinary team to assess the needs of the patient/resident
  • Significantly involved in Care Coordination meetings
  • Ensures accurate and timely MDS assessments according to state and federal regulations.
  • Ensures and completes accurate coding of the MDS assessment with information obtained via medical record review, observation and interview with center staff, patients, residents and family members.
  • Maintains the tracking system of MDS assessment schedules (time frames and due dates).
  • Coordinates Care Plan conferences with the interdisciplinary team, patient, residents and families.
  • Obtain, review and maintain all State and Federal reports, making appropriate corrections timely.
  • Monitors Quality Measures and ensures that MDSs are accurate to support and reflect the Quality Measures.
  • Provides education related to the RAI Process
  • Coordinates and completes electronic submission of required documentation to the State data base and other entities per company guidelines and State and Federal regulations.
  • Ensures Medicare and Medicaid regulatory guidelines are completed accurately and timely (i.e.: certifications, denial letters, skilled documentation, coverage criteria, etc…)
Qualifications
  • Two years of MDS experience preferred, but not required
  • Must hold current registered nursing license in the state of employment.
  • Experience inLong Term Care preferred.
  • Computer skills such as Data Entry and Word processing required. Working knowledgeof the MDS 3.0 (current version of minimum data set).
  • Must be knowledgeable of nursing and medical practices and procedures, as wellas laws, regulations and guidelines that pertain to skilled nursing facilities.
  • Mustpossess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care.
