Job summary

Finger Lakes Health is seeking a detailed, experienced MDS Coordinator (RN) to join the Living Centers in Geneva, NY. You will lead MDS assessments, care planning, and reimbursement activities to ensure regulatory compliance while supporting high-quality resident care.

This role is ideal for an RN with strong clinical knowledge, accuracy, collaboration, and a commitment to long-term care excellence in a supportive team environment.

Qualifications

• New York State RN license is required.
• Experience with MDS, care planning, and reimbursement documentation.
• Knowledge of NYSDOH regulations and survey process.

Responsibilities

• Coordinate and oversee all MDS assessments in accordance with federal and New York State regulations.
• Develop, manage, and communicate MDS schedules with the interdisciplinary care team.
• Ensure timely and accurate completion and submission of MDS, CAAs, and care plans.
• Monitor and interpret Quality Measures to support clinical outcomes and compliance.
• Lead and support reimbursement processes, including denial management.
