Qualifications

  • Must have RN or LPN license in Illinois.
  • 1-2 years of MDS experience in a Skilled Nursing Facility preferred.

Responsibilities

Job Description

What you will do in this role: The MDS Coordinator will work under/with our Regional MDS Director to coordinate and oversee the interdisciplinary assessment and care planning process in our center. This process includes:

  • Comprehensive resident assessments & Care Plans
  • Regular communication among care team members, the resident, and family;
  • Collection and transmission of data for the purposes of quality improvement;
  • Adhering to the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements.
  • The ultimate goal of the assessment and care planning process is to promote the resident’s quality of care and satisfaction in our center.
  • You may be asked to float to the floor in times of need at the center.

Qualifications:

MDS Coordinator: Registered Nurse (RN) or Licensed Practical Nurse (LPN)

What we need from you:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Illinois
  • Current MDS 3.0 certification is ideal, but not required
  • Minimum of 1-2 years of MDS experience in a Skilled Nursing Facility
