Northern Pines Rehabilitation and Nursing in Cut Bank, MT is seeking an MDS Coordinator to join our team as a full-time professional dedicated to resident assessments and compliant care planning.

The role requires coordinating with nursing, PT, nutrition, and other departments to ensure ethical, Medicare, and regulatory guidelines are followed. RN license preferred and SNF experience is expected; background checks are required.

Qualifications

• RN license preferred.
• Experience working in a Skilled Nursing Facility.
• Must be able to pass a background check.

Responsibilities

• Oversee and facilitate resident assessments to ensure comprehensive care planning.
• Coordinate care plans with nursing, PT, nutrition, and other disciplines to meet regulatory requirements.
• Ensure compliance with Federal and State regulations during assessment and planning processes.
