Job summary

Mountain View Rehabilitation and Senior Living Center seeks an experienced MDS Coordinator RN to oversee the Minimum Data Set process for residents. You will collect and review patient information to complete the MDS assessment, ensuring accuracy and timeliness.

In this role you will collaborate with the interdisciplinary team to develop individualized care plans, provide education to patients and families, and participate in quality improvement initiatives to optimize outcomes.

Qualifications

• Must hold current Pennsylvania RN license.
• 1+ year experience in long-term care or rehabilitation.

About the Role:

As an MDS Coordinator RN, you will play a critical role in ensuring the seamless coordination of patient care by facilitating the Minimum Data Set (MDS) process. Your responsibilities will include:

• Collecting and reviewing patient information to complete the MDS assessment
• Ensuring accurate and timely completion of the MDS assessment
• Maintaining accurate and up-to-date records of patient care
• Collaborating with the interdisciplinary team to develop individualized care plans
• Providing education and support to patients and families

Requirements:

• Current RN licensure in the State of Pennsylvania
