Job summary

American Senior Communities LLC is seeking an MDS Coordinator for Rosewalk Village to oversee the Resident Assessment Instrument and the interdisciplinary care planning process, ensuring accuracy and regulatory compliance. The role relies on strong clinical judgement, leadership, and collaboration to support residents and guide the care team in a long-term care setting.

A BSN is preferred and prior nursing management experience is valued to contribute to our quality standards.

Qualifications

• Graduate of an accredited school of nursing, BSN preferred.
• Minimum of one year in nursing management in the long-term industry.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric nursing setting.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI) and the interdisciplinary care planning process.
• Ensure compliance with state and federal regulations in all assessments and plans.
• Collaborate with the interdisciplinary care team to optimize resident outcomes.

The MDS Coordinator is responsible for the overall coordination and completion of the Resident Assessment Instrument (RAI) and the interdisciplinary care planning process while ensuring compliance with state and federal regulatory requirements.

Skills Needed:

• Clinical Judgement/Assessment: Attention to detail and strong clinical assessment skills. The ability to develop plans of care that are consistent with the resident’s needs.

Requirements:
