Avantara Lincoln Park, a skilled nursing and rehabilitation facility in Chicago’s Lincoln Park neighborhood, is seeking a Registered Nurse to serve as the MDS/Clinical Coordinator. The role involves leading the management of the Minimum Data Set (MDS) and utilization management to meet federal, state, and local standards.
Responsibilities
- Keep abreast of current federal and state regulations, as well as professional standards.
- Assist with the development of comprehensive care plans for guests in coordination with the MDS, ensuring accurate and needs‑based documentation.
- Coordinate, manage, and monitor the written plan of care for each guest, identifying needs and goals.
- Assist nursing management with the coordination, management, and review of nurse’s notes to confirm adherence to care plans.
- Monitor guest status changes to ensure appropriate and timely nursing or clinical team involvement.
- Ensure MDS and support documentation accurately represent the guest and meet regulatory and audit requirements.
- Perform regular audits of documentation to assure accuracy.
- Assist nursing management with the discharge process.
- Perform administrative requirements, such as completing necessary forms and reports.
- Maintain infection control and prevention practices and standard precautions.
- Follow safety precautions when performing tasks and using equipment and supplies.
- Maintain strict confidentiality regarding sensitive health information of guests.
- Report hazardous conditions, damaged equipment, accidents/incidents, and supply issues to appropriate persons.
- Maintain guest comfort, privacy, and dignity, interacting with them in a warm, respectful manner.
- Ensure each guest receives person‑centered care.
- Answer and respond to call lights promptly and courteously in guest care areas.
- Communicate and interact effectively and tactfully with guests, visitors, families, peers, and supervisors.
- Attend and participate in departmental meetings, in‑services, and training as directed.
- Recognize and support the organization’s Quality Assurance and Performance Improvement (QAPI) efforts.
- Perform QAPI roles and responsibilities as assigned, sharing improvement opportunities with leadership.
- Support QAPI efforts verbally and non‑verbally, adjusting performance and practice in accordance with initiatives and findings.
- Participate in compliance trainings and adhere to the compliance plan.
Requirements
- Graduate from an accredited nursing program.
- Valid, unencumbered Registered Nurse (R.N.) license in the state of practice.
- CPR certification.
Preferred Experience
- At least one year of experience as an R.N. in a long‑term care setting.
- At least one year of experience as a C.N.A.
Additional Eligibility
- Knowledge and training in all aspects of the MDS process.
- Knowledge of federal, state, and local requirements and regulations.
Work Environment and Physical Demands
This role requires regular walking, standing, and occasional lifting of up to 50 lb. Working in various locations within the facility, the employee may encounter contagious illnesses and will need to use standard office equipment.
