Job summary

Allbridge Rehab and Nursing Center is seeking an experienced part-time RN MDS Coordinator to manage the MDS process, CAA submissions, and the Interdisciplinary Care Plan. Flexible hours are available to accommodate staffing needs.

The role requires an active RN license with CPR, and at least 2 years of long-term care MDS coordination experience. You will guide assessments, schedule care plan conferences, and ensure regulatory compliance while supporting a collaborative team environment.

Qualifications

• Must hold RN license and CPR.
• Minimum 2 years' experience in Long Term Care as an MDS Coordinator.
• Nursing degree from an accredited college.

Responsibilities

• Monitor and guide completion of assessments in a timely manner.
• Oversee inputting of MDS assessments and transmission to state.
• Schedule and facilitate care plan conferences; coordinate monthly QM meetings.
• Manage the care planning process.

Responsibilities of MDS Coordinator:

• Monitors and guides the completion of assessments in a timely manner
• Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of the validation
• Schedules and facilitates care plan conferences Facilitates monthly QM meetings
• Manages Care Planning Process

Qualifications of MDS Coordinator:

• Must hold a Nursing Degree from an accredited college or university
• A current, unencumbered, active RN/LPN license and CPR
• At least 2 years' experience in Long Term Care as an MDS Coordinator.

Skills and Qualities of MDS Coordinator:

• Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
• Excellent written and verbal communication skills and critical thinking abilities,
• Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
• Ability to organize and prioritize.
