Job description

MDS Coordinator/Nurse Educator (Healthcare)

Join Our Team as an MDS Coordinator & Nurse Educator!

Are you a detail-oriented nurse with a passion for both clinical accuracy and team education? Northern Lights Community is seeking a full-time MDS Coordinator/Nurse Educator to ensure top-quality care for our residents in assisted living and memory care, while supporting the growth and development of our dedicated care team.

In this dual role, you'll coordinate and manage individualized resident assessments in compliance with all state and federal guidelines. You'll also help shape a culture of continuous learning by identifying and fulfilling the educational needs of our staff.

At Northern Lights, we are guided by our values of hospitality, stewardship, integrity, respect, and humor—and we bring them to life in everything we do.

Now, Let's Talk About YOU and Why You'll Love This Role:

What You'll Do (AKA: Your Superpowers)
  1. Coordinate Accurate & Timely Resident Assessments:
  • Manage and track MDS assessments (initial, quarterly, annual, and significant change) in alignment with OBRA regulations.
  • Schedule and communicate RAI (Resident Assessment Instrument) requirements with the interdisciplinary team.
  • Ensure all assessments and related documentation are complete and compliant to support care planning and reimbursement accuracy.
  • Submit MDS/RUGS data electronically to meet Medicare, Medicaid, and OBRA requirements.

  1. Ensure Documentation Excellence & Clinical Accuracy:
  • Review nursing documentation regularly to ensure clarity, consistency, and clinical relevance.
  • Guide teams in accurate ADL documentation to support strong clinical outcomes and appropriate reimbursement.
  • Maintain an accurate and current Roster Matrix and monitor key comorbidities and NTAs for optimal coding.

  1. Educate, Empower, and Elevate Your Team:
  • Provide education to staff on MDS processes, ADL scoring, and documentation best practices.
  • Support ongoing clinical training, mentoring, and professional development for team members.
  • Collaborate with the Director of Nursing to address additional educational needs and enhance staff competency.

  1. Be a Cultural Champion:
  • Embody and promote our values: hospitality, stewardship, integrity, respect, and humor.
  • Create an environment where teamwork, accountability, and joy are part of the daily rhythm.
  • Support both residents and staff with compassion, clarity, and a commitment to excellence.
What You Bring to the Table (Besides Clinical Knowledge & Heart)

Licensure & Experience:

  • Active RN license (Wisconsin) required.
  • Previous experience with MDS coordination and knowledge of RAI/MDS 3.0 processes preferred.
  • Strong understanding of Medicare/Medicaid guidelines, RUGs/CMI scoring, and care planning required.

Skills & Traits We Value:

  • High attention to detail with the ability to meet deadlines and regulatory expectations.
  • Strong teaching, communication, and leadership skills.
  • Passion for accuracy, education, and improving resident outcomes.
  • A team-first, solutions-focused mindset—and a healthy sense of humor!
Perks & Benefits (Because You Deserve It!)
  • Full-time, day shift schedule
  • Weekends & holidays as needed (with work-life balance in mind)
  • Supportive, mission-driven leadership team
  • Ongoing education and development opportunities
  • A meaningful role in a tight-knit, purpose-filled community
Join Our Team - Here's How the Process Works:
  1. Apply online: Let us know you're ready to make a difference!
  2. Screening: A recruiter will reach out within 24-48 hours if you're a strong match.
  3. Interview: Meet with leadership to discuss how you can lead documentation and education efforts.
  4. Final interview: Connect with our care team or corporate team for a deeper discussion.
  5. The offer: If it's a great fit, we'll extend a warm welcome and an official offer.
