is in search of a MDS Coordinator - Licensed Vocational Nurse (LVN) / Charge Nurse to join our team.

As the MDS Nurse, you will be responsible for collaborating with all disciplines to conduct and coordinate the completion of the Minimum Data Set (MDS), Resident Assessment Instrument (RAI), and all associated processes. The MDS Care Plan Coordinator signs and attests to the completion of the assessment within the timeframe mandated by federal regulations and company policies. You will also manage the coordination of RUG scores with ICD-9 coding for Medicare billing and oversee the resident care planning processes.

Requirements:

• Current LVN license
• SNF experience
• Charge nurse experience
