Job summary

Cantex in Gainesville, TX is seeking a qualified MDS Coordinator to ensure accurate and compliant completion of the RAI process for Medicare/Medicaid reimbursement. You will oversee MDS assessments, documentation accuracy, care plan coordination, and regulatory compliance to maintain high-quality resident care.

Ideal candidates hold a Texas RN or LVN license with LTC experience and will complete HHSC RUG training and AANAC RAI certification within one year.

Qualifications

• Current, valid Texas nursing license (RN or LVN).
• At least 2 years of long‑term care experience preferred.
• Acknowledge Completion Certificate through HHSC RUG Online Training for Nursing Facilities.
• Must complete AANAC RAI Certification within 1 year of employment.
• Strong communication, documentation, and delegation skills.
• Ability to read, write, analyze, and interpret clinical documentation, regulations, and technical procedures.
• Ability to write reports, business correspondence, nursing progress notes, and care‑related documentation.
• Ability to present information effectively to department heads, residents, families, physicians, and regulatory partners.

Responsibilities

• Maintain compliance with State and Federal Medicare/Medicaid rules and interpretations.
• Review pre-admission paperwork to ensure medical necessity.
• Attend daily stand‑up meetings.
• Coordinate Weekly Reimbursement Meetings with Interdisciplinary Team.
• Audit clinical records to ensure documentation accuracy; educate staff on guidelines.
• Obtain and update Medicare‑qualifying diagnoses for Part A residents.
• Initiate and update physician certifications for Part A residents.
• Complete all MDS assessments within required timeframes.
• Review 24‑hour nursing reports for changes in resident condition.
• Prepare for Medicaid audits and ensure documentation readiness.
• Track resident benefit days and coordinate with Financial Manager for accurate billing.
• Achieve budgeted reimbursement rate expectations.
• Uphold Patient Care and Financial Management Systems.
• Ensure resident safety and compliance with Abuse Prevention policies.
• Perform other duties as assigned.

Job Summary

The MDS Coordinator plays a key role in maintaining high‑quality resident care, supporting interdisciplinary collaboration, and ensuring reimbursement aligns with clinical delivery and federal/state guidelines.
