White Blossom Care Center is seeking a full-time (LVN) MDS coordinator. You may be our next hire to join our team!

Position Overview

The MDS Coordinator assumes full-time administrative authority, responsibility, and accountability to coordinate the completion of the required minimum data set (MDS). They work with other members of the interdisciplinary team to develop and implement a plan of care that meets the individual needs of each resident.

Qualifications/Requirements:

1. Current active license: Licensed Vocational Nurse (LVN)
2. Work Experience: At least 1 year of experience in long-term geriatric nursing care. Experience in completing the Minimum Data Set is preferred.
3. Language Skills: Ability to read, analyze, and interpret scientific and technical information. Must communicate effectively in English, both verbally and in writing.
4. Mathematical Skills: Ability to perform basic arithmetic operations—adding, subtracting, multiplying, and dividing—in all units of measure, including whole numbers, fractions, and decimals. Must be able to work with units of weight and volume.
5. Communication Skills: Exceptional communication and customer service skills, with empathy and the ability to effectively communicate with patients, families, staff, and outside agencies.
6. Additional duties: Other responsibilities as assigned.
