MDS Coordinator Responsibilities:
  • Maintain MDS/Care plan schedule for the interdisciplinary team
  • Coordinate the preparation of MDS and Care Plans per regulatory requirements
  • Track Medicare resident benefit days and validate daily Medicare census for accurate billing
  • Respond to requests for additional information from intermediaries and handle appeals within deadlines
  • Prepare Medicare denial letters for discharged residents within Medicare guidelines
  • Evaluate documentation for Medicaid reimbursement
  • Monitor and communicate infection sources and control measures
  • Provide infection control education and participate in quality and performance improvement activities
  • Utilize computerized systems for documentation and data access
  • Provide staff orientation and ongoing mentoring on infection control
  • Follow CDC guidelines and standard precautions, including isolation protocols
  • Participate in meetings, conferences, and mandatory training
  • Maintain cultural sensitivity and foster positive team relationships
Minimum Requirements:
  • Current RN or LVN license in the state of practice
  • At least 2 years of experience in skilled nursing, with MDS and Care Plan experience
  • Proficiency in RAI/PPS processes
  • Strong attention to detail, documentation, and assessment skills
  • Good communication, organizational, and computer skills
