Position Overview

Legend Oaks Healthcare and Rehabilitation - Fort Worth is seeking an MDS Coordinator. Come join our team and start making a difference!

This role involves conducting and coordinating the development and completion of the resident assessment instrument, including the minimum data set, care assessments, and care plans in accordance with state and federal requirements. As the MDS nurse, you will participate as part of the facility's interdisciplinary team in managing patients receiving skilled services, providing direct nursing care as necessary, and supporting facility surveys (inspections) by authorized government agencies.

You will also support QAPI initiatives by developing and implementing plans to address deficiencies, communicate ARD selections to the IDT, confirm skilled physician certification, and mentor CNA staff alongside the DON in completing nursing ADL documentation.
