Kenneth Moy DuPage Care Center in DuPage County is seeking a committed MDS Coordinator to oversee federally mandated assessments, collaborate with the interdisciplinary team, and ensure accurate chart review and care planning. This role requires an Illinois RN license and MDS experience, with RAC-CT certification pursued within six months.

Qualifications

Diploma in Nursing from an accredited college or university.
One year experience working with MDS.
Current IL RN license in good standing.
AANAC RAC-CT certification required within six months (reimbursed).

Responsibilities

Coordinate data collection and input for federally mandated MDS assessments.
Monitor input for accuracy, timeliness, and reimbursement standards.
Review clinical charts and condition of residents.
Plan and coordinate care with the interdisciplinary team.
Discuss resident condition with physicians and family; lead care-plan meetings.
Complete the MDS computer database; ensure Medicare/IDPH compliance.
Stay current with regulations and professional developments.
Perform RN duties as assigned.

Responsibilities

Responsibilities Include:

Coordinates with other departments to collect and input the information required for the federally mandated assessments (MDS)
Monitors input to ensure accuracy, timeliness and compliance with reimbursement authorities and standards
Reviews clinical charts and condition of patients
Plans and coordinates resident's plan of care with other members of the interdisciplinary team
Discusses resident condition with physicians, nursing staff and family members
Completes the computer database required for the MDS
Oversees documentation to comply with Medicare or Illinois Department of Public Aid reimbursement rules
Leads care plan conferences and other interdisciplinary meetings
Interprets resident charts to accurately complete sections of the MDS
Reviews resident charts for contradictions in information and researches any contradictions to determine what is valid
Stays current with job related professional developments, regulations and standards
Recommends hires and promotions, directs and evaluates employment decisions for all assigned positions
May perform the duties of a Registered Nurse as assigned

Requirements Include

Requirements include the following experience or an equivalent combination of training and experience:

Completion of coursework for a diploma in Nursing from an accredited college or university
One (1) year experience working with MDS
This position requires possession of a current license in Illinois as a Professional Registered Nurse (RN).
AANAC RAC-CT certification is required within six (6) months of employment. The DuPage County Care Center will reimburse selected candidates the cost for initial AANAC RAC-CT certification and subsequent AANAC RAC-CT recertifications.

A Post Offer Physical Is Required.

A pre-employment background check is required.
