Job summary

Oakdale Senior Alliance, leading care community, is seeking an experienced RN with MDS coordination expertise to join Seneca Senior Living as MDS Coordinator.

In this role you will conduct comprehensive resident assessments (MDS), develop and update individualized care plans, ensure timely documentation, and collaborate with physicians, therapists, and nursing staff to maintain compliance with state and federal regulations.

Qualifications

  • RN license required.
  • Minimum of 2 years in long-term care and MDS coordination.
  • Strong communication and interpersonal skills.
  • Excellent organizational and time-management abilities.
  • Ability to work both independently and within a team.
  • In-depth knowledge of state and federal long-term care regulations.

Responsibilities

  • Coordinate and complete comprehensive resident MDS assessments.
  • Manage the entire care planning process with ongoing updates.
  • Ensure timely and accurate completion of assessments and documentation.

As our MDS Coordinator, you'll be central to our commitment to quality care, responsible for:

  • Coordinating and completing comprehensive resident assessments (MDS) in strict accordance with state and federal regulations, ensuring accuracy and timeliness.

  • Managing the entire care planning process, from initial assessment to ongoing updates, to maintain accurate and up-to-date documentation that reflects each resident's unique needs.

  • Ensuring timely and accurate completion of all assessments and documentation, meeting critical deadlines and regulatory requirements.

  • Communicating effectively and collaboratively with physicians, therapists, and other healthcare providers regarding care plans, fostering a seamless interdisciplinary approach.

  • Maintaining a strong, up-to-date understanding of state and federal long-term care regulations to ensure continuous compliance and best practices.

What You'll Bring to Our Team:

We're seeking a skilled, detail-oriented, and compassionate RN with specific MDS expertise:

  • Registered Nurse (RN) license required.

  • Minimum of 2 years of proven experience in long-term care and MDS coordination.
