Qualifications

• Current and valid New Jersey RN license.
• Prior MDS Coordinator experience required.
• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, Medicare and skilled nursing regulations.
• Experience with PointClickCare preferred.
• Strong organizational skills and attention to detail.
• Ability to manage assessment deadlines and competing priorities.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.
• Develop and maintain assessment schedules for Medicare, Medicaid, and OBRA residents.
• Ensure compliance with federal and New Jersey regulations.

• Monitor Medicare eligibility, skilled coverage, and assessment requirements.

• Monitor quality measures and participate in audits, surveys, and utilization-review activities.
