Muskego Health and Rehabilitation Center is seeking a compassionate MDS Coordinator to join our team. This role combines clinical and administrative duties to assess resident health, collect and analyze data, and develop individualized care plans aligned with each resident’s needs and expectations.
This full-time position operates Monday–Friday daytime hours, requiring RN licensure in good standing and prior MDS experience in long-term care.
Muskego Health & Rehabilitation Center is seeking a compassionate, professional MDS Coordinator to join our team. As our MDS Coordinator, you will play a key role in assessing resident health, collecting and analyzing data, and developing individualized care plans that align with each resident’s needs and expectations. This position is ideal for an experienced nurse who enjoys a combination of clinical and administrative responsibilities, with a focus on quality care and compliance.
This position is full-time working Monday - Friday day hours.
Registered Nurse (RN) in good standing
Previous MDS experience in long-term care is preferred
Strong communication, organization, and analytical skills
Job Responsibilities include but not limited:
Conduct comprehensive assessments of residents
Develop and update resident care plans
Monitor patient care through assessments, observation, and medical record reviews
Collaborate with clinical teams to ensure accurate documentation and effective care delivery
Create and manage medical codes for billing and recordkeeping
