MDS Coordinator Summary of Responsibilities

The MDS Coordinator is responsible for the overall supervision, coordination, maintenance, and timely completion of the MDS, care plans, and medical records for residents.

Essential Job Functions
  1. Verifies all MDS for both Medicare and Long Term residents are accurate and completed in a timely manner.
  2. Supervises subordinate MDS coordinators.
  3. Ensures that all assessments are accurate and completed in a timely manner.
  4. Oversees the monthly Triple Check meeting and ensures it is scheduled according to accounting needs.
  5. Coordinates, conducts, and completes the MDS, resident care plans, and conferences on the computerized system.
  6. Assures accuracy and completeness of the entire medical record within HFCA and Medicare standards and guidelines.
  7. Ensures MDS electronic submission occurs per State of Texas regulations.
  8. Participates in family/resident care plan conferences.
  9. Ensures effective communication with other shifts and departments regarding medical record documentation.
  10. Notifies physicians and family of changes in residents' conditions.
  11. Encourages rehabilitative techniques to restore residents' independence.
  12. Assists DON in developing and implementing medical records policies and procedures.
  13. Audits resident charts as assigned.
  14. Coordinates the restorative program.
  15. Presents and attends in-service training and education sessions as assigned.
  16. Performs specific work duties and responsibilities assigned by supervisor.
  17. Ensures timeliness of physicians' visits.

  • Experience with MDS transmittal process and related regulations.
  • In-depth experience with MDS software.
