MDS/CARE PLAN COORDINATOR
SUMMARY: The MDS/Care Plan Coordinator is to complete the RAI process for each resident of the facility, as required by State and Federal regulations. This includes transmission and verification of receipt by the State. Also, maintain compliance with our own policies and procedures. Ensure that the highest degree of quality care is maintained at all times. As the MDS/Care Plan Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary to carry out your assigned duties.
