Job summary

The Goodman Group, LLC is seeking an RN MDS Coordinator to lead the development and execution of the MDS/RAI process in compliance with state and federal regulations within our Florida facility.

The role requires coordinating with the interdisciplinary team to ensure accurate assessments, compliant care plans, and ongoing staff training. The RN license in Florida is required, with preferred LTC experience.

Responsibilities

The MDS Coordinator (RN) plays an integral role in the success of our team, our community and creating a place our residents are proud to call home. The primary purpose of this position is to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI) process in accordance with the state and federal requirements and the policies and goals of the facility.

Essential Job Functions
  • Conduct and coordinate the development and completion of the Minimum Data Set (MDS) assessment in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation and completion of Care Area Assessment’s (CAA) and Care Area Trigger’s (CAT).
  • Manages the reimbursement process and works collaboratively with the business office and therapy department.
  • Assist and provide input into ongoing updates of policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Ensures facility is in compliance with the Resident Assessment Instrument (RAI) process.
  • Work with the Interdisciplinary Team (IDT) in completing a comprehensive MDS and care plan for each resident.
  • Ensure that care provided is in accordance with the resident's wishes and interests.
  • Develop and participate in the planning, conducting, and scheduling of timely in-service training classes that assist in nursing to ensure there is good supporting documentation for the MDS.
  • Assist in developing any training activities needed concerning resident assessment/care plan skills.
  • Coordinate the development of a written plan of care (immediate and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which staff are responsible for each element of care.
  • Monitor the care plan so that it includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment.
  • Assist the Director of Nursing (DON) and relevant supervisors of other departments in ensuring that all personnel involved in providing care to the resident are aware of the resident's care plan.
  • Monitor and ensure compliance of the MDS process.
  • Coordinate the review and revision of the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, ensuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status.
Personnel Functions
  • Perform employee performance evaluations (if applicable).
  • Determine MDS staffing requirement, and making recommendations to the DON concerning employee dismissals, transfers, etc.
  • Review and evaluate your team’s work process and work to improve.
  • Develop work assignments and/or assist in completing and performing such assignments.
  • Submit complaints and grievances DON.
Knowledge And Critical Skills
  • Be able to make independent decisions and follow instructions.
  • Deal tactfully with personnel, prospects, residents, family members, visitors, government agencies/personnel, and the public.
  • Capable of working with ill, disabled, elderly, and emotionally upset people within the facility.
  • Communicate effectively in a manner that is sufficient for supervisors, team members, prospects, residents, and families.
Education And Experience
  • Must meet all applicable state and federal requirements for this position.
  • 1 year of experience in the RAI process.
  • 2 years long term care facility care experience preferred.
  • Possess a current and active license to practice as an RN in this state.
Supervisory Responsibility

This position may have direct reports or supervisory requirements.

Working Conditions and Physical Demands

Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.

Physical Requirements
  • Prolonged periods of sitting at a computer.
  • Ability to lift fifty (50) pounds. Moving, lifting or transferring of residents may involve lifting up to 75 pounds
  • Ability to exert maximum muscle force to lift, push, pull or carry patients
  • Ability to use abdominal and lower back muscles over time without fatigue

  • Ability to bend, stoop, stretch, twist, sit and reach
