Job Details

Location: Rochester, NY

Salary Range: $33.00/hr - $35.00/hr

Responsibilities

1. Completing accurate assessments, MDS & care plans as assigned
2. Initiating care plans and supporting activities as assigned
3. Creating and distributing monthly care plan calendars in a timely fashion
4. Maintaining & updating all care plans and assessments as required
5. Monitoring & auditing clinical records, ensuring accuracy & timeliness
6. Informing DON of persistent issues related to non-compliant documentation
7. Protecting the confidentiality of Resident & Facility information at all times

Requirements

1. Prior MDS 3.0 experience
2. Valid New York RN License
3. Long Term Care experience required
4. Highly organized, professional, and motivated
5. Solid computer skills
6. Excellent communication skills
7. Friendly and a team player
