Job summary

CareCore at Willowood in Ohio is seeking an experienced MDS Coordinator. You will ensure accurate MDS assessments, regulatory compliance, and quality resident outcomes while collaborating with an interdisciplinary team.

This onsite role requires strong clinical knowledge, attention to detail, and the ability to educate staff on MDS processes to support optimal reimbursement and resident care.

Qualifications

• Active RN or LPN license is required.
• Prior MDS Coordinator experience in long-term care.
• Familiarity with RAI, PDPM, Medicare/Medicaid.
• Strong organizational and critical thinking abilities.

Responsibilities

• Complete and coordinate MDS assessments per regulations.
• Ensure timely care plans and documentation.
• Collaborate with nursing, therapy, dietary, social services.
• Monitor documentation for reimbursement and compliance.
• Support quality initiatives and survey readiness.

This is an onsite/ non-remote position.

As our MDS Coordinator, you will play a vital role in ensuring accurate assessments, regulatory compliance, and quality resident outcomes. This is an excellent opportunity for a detail-oriented nurse who enjoys working closely with an interdisciplinary team and making a meaningful impact every day.

Responsibilities:

• Complete and coordinate MDS assessments in accordance with state and federal regulations.




• Ensure accurate and timely completion of care plans and required documentation.




• Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.




• Monitor clinical documentation to support appropriate reimbursement and regulatory compliance.




• Participate in quality improvement initiatives and survey preparedness.




• Educate and support staff regarding MDS processes and documentation requirements.

Qualifications:

• Active LPN or RN license required. RN preferred.




• Previous MDS Coordinator experience in a long-term care or skilled nursing setting required.




• RAC-CT certification preferred.




• Working knowledge of Medicare, Medicaid, the RAI process, and PDPM.




• Strong assessment, organizational, and critical thinking skills.




• Experience with PointClickCare (PCC) preferred.




• Excellent communication and teamwork abilities.
