Job summary

Adara Living seeks an MDS Coordinator to lead resident assessments (MDS) and care planning in a full-time capacity in Broomfield, CO. The role requires an RN license, geriatric experience, and CPR certification.

Qualifications

• Bachelor's degree in nursing from an accredited college or university.
• Active Colorado RN license.
• Minimum of two years nursing experience; geriatric nursing preferred.
• CPR certification required or obtained upon hire.

Responsibilities

• Coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines; including RAPs and Triggers.
• Develop preliminary and comprehensive nursing plans of care for each resident.
• Attend daily department meetings.
• Coordinate the development of a written care plan identifying problems/needs, care to be given, goals, and responsible professionals.
• Ensure care plans include measurable objectives and timetables for medical, nursing, and psychosocial needs.
• Ensure initial resident assessment is completed within fourteen days of admission.
• Perform other duties as specified by the supervisor.

Salary Range: $50.00 To $55.77 Hourly

We are looking for a compassionate, patient, and energetic Registered Nurse to join our Nursing Department as the MDS Coordinator. Position Summary:

• Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers.
• Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and facility policies.
• Attends daily department meetings
• Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
• Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment.
• Ensure that an initial resident assessment is completed within fourteen days of the resident's admission.
• Other duties as specified by the supervisor.

Requirements:

• Bachelor's degree in nursing from an accredited college or university
• Currently licensed/ registered in the state of Colorado. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
• Minimum of two (2) years nursing experience. Geriatric nursing experience preferred.
• CPR certified or obtained upon hire. CPR certification must remain current during employment.
