Job summary

Phoenix Health Services LLC in Akron, OH is seeking a Licensed Practical Nurse or Registered Nurse to perform resident MDS assessments, document findings, and determine referrals for further care. Qualifications

  • Must hold a current, unencumbered license to practice as a Licensed Practical Nurse or Registered Nurse in Ohio.
  • Experience with MDS assessments and CMS guidelines is preferred.
  • Knowledge of nursing practices, regulations, and care facility guidelines.

Responsibilities

  • Assess residents’ physical and mental function and document on MDS forms completely and accurately.
  • Determine referrals to other health care professionals as needed.
  • Enter and print MDS assessments and related reports from the computer program and state reporting sites.
  • Participate in Quality Assurance and interdisciplinary team meetings.
  • Plan, organize, implement, and interpret the plan of care and conduct care conferences.
  • Communicate effectively with nursing staff, residents, families, and physicians.

BASIC FUNCTION

The primary purpose of your job is to assess residents’ physical and mental function and document on minimum data set forms completely and accurately, document all additional assessments required completely and accurately. determine appropriate referrals to other health care professionals and to use the resident assessment protocols to determine whether to proceed or not proceed

Administrator, Director of Nursing

Essential Job Functions

The employee must be able to perform each essential job function effectively to be successful in this position.

  • Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse or Registered Nurse in this state.
  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Must demonstrate the skills necessary to interview residents, residents’ representatives and others with information related to the minimum data set assessment.
  • Must demonstrate the skills necessary to examine residents to collect information related to the minimum data set assessment.
  • Must possess the ability to enter data into the computer program for minimum data set assessment.
  • Must possess the ability to print the minimum data set assessment(s) and related reports from the computer program and the state reporting sites.
  • Must possess the skill and ability to review reports of timely completion and scoring of the minimum data set assessments and care plan and take follow-up action as necessary.
  • Must participate in the Quality Assurance program, for the nursing department, and cooperate with the Quality Assurance Coordinator.
  • Must participate and/or conduct the interdisciplinary team meetings and rehabilitation utilization meetings.
  • Must demonstrate ability and understanding to complete minimum data set assessment schedule and communicate it to the interdisciplinary team.
  • Must demonstrate completion of assignments accurately and adhere to established time schedules set forth by CMS.
  • Must possess comprehension and be able to demonstrate ability to reconcile end of month/end of quarter reports.
  • Must demonstrate ability to establish open lines of communication between all nursing personnel.
  • Must possess ability and the willingness to work harmoniously with other personnel.
  • Must possess the ability to plan, organize, develop, implement and interpret the plan of care process and coordinate the care conference meetings.
  • Must demonstrate a good understanding of case mix and maximization for reimbursement.
  • Must demonstrate ability and understanding of medical diagnostic coding.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities.
  • Must consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, residents, families, therapy staff and consultants.
  • Must be able to relate information concerning a resident’s condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
