Job summary

Cypress Valley Center for Nursing and Rehabilitation is seeking an experienced MDS (Minimum Data Set) Coordinator to oversee the MDS coordination and ensure alignment with Federal and State regulations. The role includes coordinating with the Interdisciplinary Team, tracking assessment schedules, and leading Medicare utilization and billing-related meetings.

The position requires an RN licensed in NC, at least three years of clinical experience (long-term care preferred), and CPR certification.

Qualifications

• RN licensed in North Carolina.
• Minimum 3 years clinical experience in health care; long-term care preferred.
• Must have CPR certification.
• Demonstrates integrity and professionalism with interdisciplinary team, patients and families.

Responsibilities

• Oversee MDS coordination and completion with MDS, CAA's and Care Plans per Federal/State regs.
• Assist in MDS completion with Interdisciplinary Team.
• Notify Interdisciplinary Team of MDS assessment schedule for all payer sources and changes.
• Lead weekly Medicare utilization meetings and month-end meetings for federal billing compliance.
• Collect information for MDS from medical records, bedside assessments, and interviews.
• Develop and monitor a system to ensure timely, signed assessments by all team members.
• Observe nursing care, review documentation, and make recommendations; assist with chart audits.

Job Summary

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations.
• Assist in completion of the Resident Assessment Instrument with the Interdisciplinary Team.
• Notify all Interdisciplinary Team members of the MDS Assessment schedule for all payer sources and any changes related to payer source.
• Lead weekly Medicare utilization meetings, and month end meetings to assure federal billing requirements are met.
• Information to complete the MDS is to be collected using the medical record, bedside assessment, and staff, resident and/or family interviews.
• Develop and monitor a system to verify that all Interdisciplinary Team members have completed, dated, and signed the assessments according the Federal Regulations.
• Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.

Requirements

• Registered Nurse with NC.
• Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred.
• Must have a current/active CPR certification.
• Personal integrity and professionalism to work effectively with the Interdisciplinary Team, Patients, and Families.
• Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
