Job summary

AmeriHealth Caritas is seeking a Maternity Case Manager to support pregnant members through care coordination and case management, ensuring optimal health during pregnancy and postpartum.

The role targets experienced nurses with a passion for maternal health and community care, offering flexible work options and strong benefits. Kannapolis, NC-based, full-time schedule with Monday–Friday hours.

Qualifications

• Associate degree in nursing is required; bachelor degree preferred.
• 3+ years maternity RN experience including labor and delivery and high‑risk antepartum/postpartum care in an acute care setting.
• 3+ years case/care management experience with Medicaid populations in homecare or community-based settings preferred.

Responsibilities

• Assess members to determine care coordination and management needs.
• Provide support during pregnancy and postpartum.
• Complete assessments including health history and social determinants of health.
• Coordinate physical, behavioral health, and social services.
• Manage medications including reconciliation and adherence support.
• Identify problems/barriers for care coordination and implement interventions.
• Create a plan of care to achieve optimal health.
• Schedule follow-up calls and referrals.
• Document progress toward goals and resolving issues.

About This Role

The Maternity Case Manager provides support to pregnant members through care coordination and case management services, helping them achieve optimal health during their pregnancy and postpartum period. This role is ideal for experienced nurses passionate about maternal health and community care.

Highlights

• Location: Kannapolis, NC
• Schedule: Full-time, Monday through Friday, 8:30 AM EST to 5:00 PM EST

What You'll Do

• Assess members to determine care coordination and management needs.
• Provide support to members throughout their pregnancy and postpartum period.
• Complete comprehensive person-centered assessments, including health history and social determinants of health.
• Coordinate physical, behavioral health, and social services.
• Manage medications, including regular medication reconciliation and adherence support.
• Identify problems/barriers for care coordination and implement appropriate interventions.
• Create a plan of care to assist members in achieving optimal health.
• Identify and implement appropriate interventions based on member needs and clinical progress.
• Schedule follow-up calls and make appropriate referrals.
• Document progress towards meeting goals and resolving issues.

Requirements

Education

• Associate Degree in Nursing required; Bachelor Degree preferred

Experience

• Minimum of 3 years of maternity experience as a RN, including labor and delivery and high‑risk antepartum and postpartum care in an acute care setting
• Minimum of 3 years of Case/Care Management experience working with Medicaid populations in a homecare or community‑based setting preferred

Licenses & Certifications

• Active and unencumbered Registered Nurse license in North Carolina
• Valid driver’s license with car insurance
