Job summary

The Helen Ross McNabb Center is seeking a MAT Peer Recovery Support Case Manager to provide client-centered case management for individuals with co-occurring disorders in rural counties served by the MAT mobile clinic.

Primary duties include coordinating care, documenting in EMR, and collaborating with a multidisciplinary team to support recovery, transportation access, crisis intervention, and treatment planning.

Qualifications

• State of Tennessee CPRS training is required or willing to obtain.
• Experience with substance use disorders and co-occurring disorders.
• Understanding of trauma-informed care, suicide prevention, crisis intervention and overdose reversal.

Responsibilities

• Provides monthly face-to-face case management contacts for clients with substance use and/or co-occurring disorders.
• Submits 75+ contacts per month for MAT clients via mobile clinic.
• Coordinates care with prescribers, nurses and therapists within a multidisciplinary team.
• Ensures care plans are developed with client, family and provider signatures.
• Attends weekly multidisciplinary treatment team meetings and staff meetings.

JOB PURPOSE/SUMMARY

Summary of role of team:

• The ED Peer Recovery Support Specialist will act as a member of a multi-disciplinary team, providing support to assist clients, family members and other parties affected.

Summary of position:

• This position will provide case management support to clients experiencing co-occurring disorders in rural counties.

TYPICAL WORKING CONDITIONS/ENVIRONMENT

• This position will involve client contacts directly in the community.
• This position will partner with our mobile clinic to provide care to rural populations.

JOB DUTIES/RESPONSIBILITIES

1. Quality Care

• Provides monthly face-to-face case management contacts for clients experiencing substance use and/or co-occurring disorders.
• Submits a minimum of 75 contacts per month for clients receiving MAT services from our mobile clinic.
• Provides on-going daily support to the prescriber/nurse/therapist in relation to any client's needs as part of a multidisciplinary team.
• Evidences coordination of care for clients with intra/inter-agency programs, courts and other stakeholders.
• Assists client with accessing transportation and provides transportation when other viable options are not available to ensure continuity of care.
• Assists clients with accessing insurance/Safety Net/ prescription coverage.
• Helps RN facilitate case management between the primary care provider, behavioral health provider, pharmacy, specialists, and/or natural supports regarding health and recovery needs of the client.
• Promotes recovery skills education with the client and/or family.
• Provides crisis intervention and emergency services as needed.
• Completes care plan within first three contacts with client and ensures provider, family and client participation in treatment planning as evidenced by signatures on care plans.
• Attends and is timely to weekly multidisciplinary treatment team meetings.
• Attends and participates in administrative/ staff team meetings without tardiness.

2. Documentation requirements

• Completes all required clinical documentation in accordance with Center Policy and Procedure and funding source guidelines.
• Completes required documentation entries in the State TnWits system.
• Completes and signs EMR progress notes within two business days of encounter.
• Ensures that all clients have an up-to-date ROI, care plan, crisis plan, consent to contact after discharge, DLA-20, and updated consents and signature admissions page every 6 months and/or as needed.
• Creates and coordinates treatment interventions that are reflected in progress note documentation based on the care plan goals.
• Provides completed Client Contact Logs to program coordinator by date assigned.
• Attends supervision with program supervisor.

COMPENSATION:

• Starting salary for this position is approximately $17.97/hr based on relevant experience and education.

Schedule:

• Weekday hours.
• Weekends and holidays off.
• The position may start as early as 8:30am and may end as late as 5:30pm.
• MAT clinic runs late on Tuesday evenings.
• On Tuesdays, this position may be required to work a 10:30-6:30pm shift.

Travel

• This position will require utilizing a personal dependable vehicle to conduct Center business.
• It will require travel in rural counties which may include the following: Grainger, Cocke, Jefferson, Claiborne, Hancock.
• Maintaining a dependable vehicle and certified driver status is a condition of employment.
• Staff members may be required to provide transportation in their personal vehicle and staff members are reimbursed for use of their vehicle for this purpose.

Equipment/Technical Competency

• This position does require computer skills for timekeeping, scheduling and use of electronic medical records entry, and familiarity with Microsoft Office Products (Word/Excel/Outlook).

QUALIFICATIONS - MAT Peer Recovery Support Case Manager

Experience / Knowledge:

• Has or is willing to obtain the State of Tennessee Certified Peer Recovery Specialist Training (CPRS).
• Must have coursework and/or experience and understanding in the areas of cultural diversity, human development, and treatment of substance use disorder, understanding of trauma-informed care, suicide prevention, crisis intervention referral and knowledge of overdose reversal.

Education / License

• A bachelor's degree preferred in a health-related field of counseling, psychology, social work, or three years' experience working with individuals with substance use disorders, or co-occurring diagnoses preferred.
• A minimum of HS Diploma or GED.

Physical/Emotional/Social -Skills/Abilities:

• Exposure to biological hazards.
• Hearing of normal and soft tones.
• Valid driver's license (F-endorsement).
• Lifting up to 50 lbs.
• Pushing/pulling up to 150 lbs.
• Frequent sitting, standing, walking, bending, stooping, and reaching.
• Required to be certified in, and effectively implement, verbal and physical de-escalation techniques that include a wide range of bodily movements; including but not limited to, grasping, holding another person, running, and walking.
