NaphCare seeks a MAT Nurse Practitioner to deliver Medication-Assisted Treatment for incarcerated individuals in Pueblo, CO. You will conduct assessments, initiate Suboxone therapy, and coordinate care with a multidisciplinary team in a correctional setting.

Qualifications

• Advanced nursing education required (MSN or DNP).
• Active NP license in Colorado and DEA registration.
• MAT/OUD treatment experience is required.
• Collaborative agreement with on-site physician is required.
• CPR certification (BLS/AHA) required.
• Board certification in a specialty through ABMS preferred.

Responsibilities

• Provide MAT services for incarcerated individuals with OUD, focusing on buprenorphine/naloxone.
• Conduct initial and ongoing clinical assessments for OUD and comorbidities.
• Initiate, monitor, and adjust Suboxone therapy per guidelines and policies.
• Manage induction, stabilization, maintenance, tapering phases of MAT.
• Collaborate with correctional healthcare, custody, and mental health teams.
• Monitor adherence, response, and diversion risks in a correctional setting.
• Educate patients on OUD, MAT, relapse prevention, and post-release care.
• Document accurately in EHR, including MAT reporting and regulatory needs.
• Participate in discharge planning and community MAT referrals.
• Maintain compliance with controlled substances and correctional standards.

Overview

NaphCare is hiring a MAT Nurse Practitioner to join team of medical professionals in the Pueblo County Jailin Pueblo, CO.

Responsibilities

The MAT Nurse Practitioner plays a critical role in our continued mission to provide quality care that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and are not limited to:

• Provide comprehensive Medication-Assisted Treatment (MAT) services for incarcerated individuals with Opioid Use Disorder (OUD), with a primary focus on buprenorphine/naloxone (Suboxone).


• Conduct initial and ongoing clinical assessments for OUD, including substance use history, withdrawal evaluation, psychiatric and medical comorbidities, and readiness for treatment.


• Initiate, continue, adjust, and monitor Suboxone therapy in accordance with evidence-based guidelines, DEA regulations, and correctional health policies.


• Manage induction, stabilization, maintenance, and tapering phases of MAT as clinically indicated.


• Collaborate closely with correctional healthcare staff, custody staff, mental health providers, and substance use counselors to ensure coordinated, safe, and effective care.


• Monitor patient adherence, response to treatment, and potential diversion risks within a correctional setting.


• Provide patient education on OUD, MAT, relapse prevention, overdose risk, and continuity of care upon release.


• Participate collaboratively with an interdisciplinary team to create individualized treatment plans that address medical, behavioral health, and social determinants of health.


• Ensure accurate, timely, and compliant documentation in the electronic health record, including MAT-specific reporting and regulatory requirements.


• Participate in discharge planning and continuity-of-care coordination, including referrals to community-based MAT providers upon release.


• Maintain compliance with state, federal, and facility regulations related to controlled substances, MAT delivery, and correctional healthcare standards.


• Participate in quality improvement initiatives, clinical audits, and program development related to OUD and MAT services.

Stay current with evolving best practices, clinical guidelines, and regulatory changes related to OUD treatment and MAT.

Qualifications

Education / Experience / Requirements

• Advanced nursing education (Master's or Doctorate)


• MAT/OUD treatment experience required


• CARN-AP preferred


• Current licensure as a Nurse Practitioner


• Must hold a current unrestricted license as a Nurse Practitioner in Colorado.


• Current DEA registration


• An appropriate collaborative agreement with the on-site Physician


• Must possess an active BLS CPR Certification (AHA required)


• Be certified in a Board recognized by the American Board of Medical Specialties


• Knowledge of nursing practice principles and techniques, occupational health and safety standards, infectious disease management, applicable health care laws, regulations, and procedures for direct health care services.


• Ability to maintain confidentiality of all proprietary and/or confidential information


• Excellent communication, organizational, and problem-solving skills


• Able to work independently and in a team


• Experience working in a correctional facility or acute care center is a plus


• Integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements


• Must pass the facility’s criminal background check and drug screening
