Purpose and Scope

Provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients (if applicable), utilizing Social Work Theory of Human Behavior and accepted social work practices. Collaborates with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients. Supports the Fresenius Kidney Care (FKC) commitment to quality indicators, outcomes, and quality improvement activities, including patient satisfaction and quality of life. Participates actively in process improvement activities. This is an entry-level MSW role.

Principal Duties and Responsibilities
  1. Patient Assessment / Care Planning / Counseling
    1. Assess patients' psychosocial status, strengths, and needs affecting treatment outcomes.
    2. Participate in care planning with the patient and healthcare team to identify effective interventions.
    3. Use FKC education programs, social work theories, and quality of life tools to address barriers and meet treatment goals.
    4. Monitor and intervene to help patients adjust to dialysis and improve psychosocial well-being.
    5. Provide supportive counseling within scope of training and licensing.
    6. Assist patients seeking transplant with education and counseling.
    7. Refer patients to community resources and assist with access to vocational and educational programs.
    8. Assess awareness of advance directives, facilitate discussions, and support decision-making.
    9. Document interactions and interventions per policy.
  2. Quality
    1. Provide psychosocial support addressing adherence, outcomes, and quality of life.
    2. Participate in monthly quality review meetings and report on relevant indicators.
  3. Patient Education
    1. Assess patient knowledge and barriers regarding kidney disease and treatment adherence.
    2. Provide tailored education on treatment modalities and transplant processes.
    3. Educate about patient rights, grievance procedures, and facility policies.
    4. Contribute to quality assurance activities.
  4. Patient Admission and Continuity of Care
    1. Review patient rights, policies, and address immediate needs.
    2. Support scheduling and admission processes to optimize adherence.
    3. Identify and communicate concerns related to discharge or transfer.
  5. Insurance and Financial Assistance
    1. Coordinate with insurance staff to address coverage issues.
    2. Educate patients about payment options and assist with insurance changes.
    3. Refer patients to billing solutions as needed.
  6. Staff Support
    1. Assist with staff hiring and training on psychosocial topics.
    2. Participate in team meetings and discuss urgent patient issues.
  7. Adhere to caseload guidelines and perform other duties as assigned.

  • Travel may be required for facility visits or home visits.
