East Valley Community Health Center Inc. is seeking a Behavioral Health professional to screen, assess, and connect patients to behavioral health services within our integrated care system in Pomona, CA.

Responsibilities include brief consultations, crisis intervention, care coordination, and ongoing documentation while maintaining a weekly caseload. Strong communication and EHR skills are required. DOE salary; benefits offered.

Qualifications

• Master’s degree in Social Work and CA Associate Clinical Social Worker license.
• Two years social work experience in medical or mental health setting.
• Bilingual English/Spanish and bicultural.
• Strong interpersonal, teamwork and communication skills.

Job description

The purpose of this position is to work within an integrated health care system ensuring that patients seeking medical services are screened for mental health conditions such as anxiety, depression and substance use and that their mental health needs are addressed.

MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:

Ensure that new patients and as appropriate, returning patients complete required screening tools;

• Score behavioral health screeners and enter results into EHR.
• Perform brief consultation with patients during medical visit for enrollment and or linkage in Behavioral health services.
• Work closely with medical providers and care teams to intervene with patients needing immediate consultation.
• Perform crisis intervention, case management, referrals and follow-up as required;
• Develop a collaborative relationship with care teams, medical providers and other BH staff members.

Participate in weekly and monthly staff meetings with care teams, BH department and supervisor.

• Complete all required documentation within 24 time period.
• Participate in clinical supervision meetings;
• Maintain weekly caseload of 20 patients enrolled in therapy services.
• Facilitate Support Groups.
• Assists and/or completes additional tasks as assigned.

POSITION REQUIREMENTS AND QUALIFICATIONS:

• A Master’s degree in Social Work and current license as an Associate Clinical Social Worker issued by the California Board of Behavioral Science Examiners

Two years social work experience in a medical or mental health setting.

• Must be bilingual (English/Spanish) and bi-cultural.
• Strong interpersonal skills. Must be able to work within a team

Must have excellent written and verbal communication skills.

• Computer literacy in word processing and spreadsheet programs.
• Must be familiar and have the ability to work within an Electronic Health Records system

DOE: $31.00 - $34.54
