Qualifications

• MD or DO medical degree.
• Post-residency clinical experience (2+ years).
• Recent value-based care experience (1+ year).
• Board certification or eligibility in Internal Medicine or Family Medicine.
• Strong collaboration and leadership of care teams.
• Ability to deliver high-quality primary care with accessibility.
• Passion for elderly and underserved populations.
• Internal candidates must be in good standing with no disciplinary action in the past 12 months.

Responsibilities

• Render outpatient primary care and consulting services.
• Maintain accurate patient records and documentation.
• Document care and ensure data quality in records.
• Collaborate with care team to provide high-quality care and morale.
• Ensure timely evaluation of urgent needs and minimize wait times.
• Participate in community outreach and PR activities.
• Attend clinical meetings and review outcomes and quality metrics.
• Provide call coverage as needed per ArchWell Health schedule.
