Essential Functions:

1. Assist patients with activities of daily living, including personal hygiene, ambulation/transfers, dressing/shaving.
2. Prepare meals and snacks as instructed.
3. Accompany patients on errands or medical appointments.
4. Perform light housekeeping activities.
5. Follow patient-specific care plans, including hospice support, medication reminders, and range of motion exercises.
6. Assist with blood glucose monitoring, care of bed-bound patients, incontinence care, and use of lifting equipment such as gait belts and Hoyer lifts.
