Job summary

Qualifications

Responsibilities

• Serve as a clinical resource for staff.
• Supervise and facilitate development of staff and oversee day-to-day department tasks.
• Ensure staff are competent in the use of InterQual to progress the plan of care.
• Collaborate with physicians, nurses, and other care team members for timely patient management.
• Monitor the quality of CM interactions with patients, families, physicians and the care team.
• Audit case managers' documentation for quality and compliance.
• Participate in clinical performance improvement activities and communicate with patients and staff.
• Oversee scheduling, staffing, education and competencies of the department.
• Complete reports as assigned by the Director of Case Management.
• Seek ways to control costs without compromising patient safety or care quality.
• Adhere to policies, standards and escalation procedures.
• Demonstrate knowledge of regulatory requirements and ethics/compliance policies.

Job Summary and Qualifications

The Manager of Case Management Services is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for their assigned area of responsibility in alignment with the goals of the department.

What you will do in this role:

• Serve as a role model and functions as a clinical resource for staff.

• Supervise and facilitate development of staff and oversees the day-to-day tasks/activities in the department.

• Ensure staff are competent in the use of InterQual to progress the plan of care.

• Work collaboratively and maintain active communication with physicians, nurses, and other members of the interdisciplinary care team to effect timely, appropriate patient management.

• Monitor the quality of CM interactions with patients, families, physicians, and the multidisciplinary team by regularly shadowing staff to ensure clear understanding and adherence to department processes.

• Monitor quality of documentation and assessments by regularly auditing the case managers’ documentation.

• Participate in clinical performance improvement activities. Communicate effectively with patients, family, medical staff, care team members, students, supervisors, and outside contacts.

• Assume responsibility for scheduling, staffing, education, and competencies of department.

• Complete daily, weekly and monthly reports as assigned by the Director of Case Management.

• Seek ways to control costs without compromising patient safety, quality of care, or the services delivered.

• Adhere to established policy and procedure and standards of care; escalates issues promptly through the established chain of command.

• Demonstrate knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives.

• Serve as an advocate for patient's rights, needs, and values; ensure that patients’ ethnic, cultural, or religious values, beliefs, preferences, and needs are considered and aligned.

What qualifications you will need:

• Master of Social Work (MSW), CSWE accredited required.

• Certification in social work or case management preferred.

• 3 years of experience in acute care social work preferred.

• 2 years of supervisory/leadership experience preferred.
