Job summary

Petaluma Valley Hospital is seeking a dedicated Case Manager to join our team. You will coordinate whole-person care across the continuum, collaborating with patients, families, nurses, social workers, physicians, payers, and the care team to optimize transitions and outcomes.

You will address social determinants of health and real-world barriers to care, ensuring respectful shared decision-making. This role emphasizes a patient-focused culture with commitment, understanding, and mutual respect

Qualifications

• Hold a Bachelor's in Nursing and CA RN license.
• 5 years of healthcare experience.
• 3 years of Case Management in an acute care setting.
• 1 year of healthcare leadership experience.
• CCM or ACM certification preferred.

Responsibilities

• Drive the Case Management process: assessment, planning/intervention, implementation, care coordination, monitoring, and evaluation.
• Advocate for optimal health outcomes, appropriate resource utilization, and access to services.
• Collaborate with post-acute care providers to ensure patients' needs are met within the community.
• Support achievement of Providence's Mission of caring for everyone, especially the most vulnerable.

Job description

We are seeking a dedicated Case Manager to join the team at Petaluma Valley Hospital. Case Management here is a collaborative practice — working alongside patients, caregivers, nurses, social workers, physicians, payers, and the broader care team to coordinate effective, whole-person care across the continuum.

You'll play a key role in facilitating communication and transitional care, honoring each patient's right to self-determination while addressing the social determinants of health and the real-world complexities of care — from barriers to coping and mental health, to substance use and psychosocial needs.

What You Bring

Required Qualifications

• California Registered Nurse License upon hire
• 5 years of experience in a healthcare role
• 3 years of Case Management experience within an acute care setting
• 1 year of healthcare leadership experience

Preferred Qualifications

• Master's Degree in Healthcare or related field
• Certified Case Manager (CCM) or Accredited Case Manager (ACM) upon hire
